    Abstract : Background : The present study is undertaken to acquire normative data of health-related physical fitness (HRPF) of Republic of Korea Air Force (ROKAF) aircrews and to provide norm-referenced standard of HRPF. Methods: From 15 May 1998 through 14 December 1999, 1284 KAF pilots are checked muscular strength, muscular endurance, flexibility, and body fat percentage. Result : The grip strength was relatively maintained, but flexibility, back strength, and muscular endurance were declined according to promotion and aging. And the incidence of obesity determined by bioelectrical impedence analysis was higher than that assessed on the basis of height and weight. Conclusion : It is suggested that many pilots who were normal in BMI have abdominal or visceral obesity. Further studies must be made on the health promotion program for the pilots, on the changes in physical fitness, and on the criterion-referenced standard of HRPF of the pilots.

    Abstract : Background: It is known that depression and anxiety tendency causes physical or social dysfunction. Nowadays, the air travel is increasing as the life condition has been improved so the health of pilots who are in charge of passengers become a very important issue. However, few studies have been done on this issue, especially the relationship between depression & anxiety and medical diseases among them. This study was performed to know the factors affection pilots' sick leave and the correlation between depression & anxiety and sick leave. Methods: Self administered questionaires were obtained from 123 pilots of one of the airline companies at Seoul from 1st April to 30th may 2003. Depression tendency was measured based upon BDI, the level of anxiety was measured by the standardised STAI and obesity was diagnosed if a BMI is over 25 kg/m2. The data on the subjects' age, position, airlines and the days of sick leave were obtained from personal record. Results: All the study subjects were male with the average age of 40.23±7.48 (ranging from 30 to 60). The average age for those who had sick leave was 34.58±3.50 and that for those who had not was 40.48±7.55 and these two average ages showed difference with statistical significance (P value <0.001), and the odd ratio of sick leave about age was 0.796 (95% CI: 0.645-0.938). Whether a pilot had sick leave or not, however, was not statistically related to the level of BDI, STAI, flying duty, flying route, and body mass index (P value >0.05). Conclusion: This study shows that the average age of sick leave group was lower than that of non-sick leave group (P<0.001). The level of depression and anxiety were not related to the sick leave. The reasons for this seems to be the fact that pilots are healthier than general population and also they pay more attention to their own health. The younger pilots tended to have more days of sick leave, which might be explain that the older pilots were more cautious about the disadvantages of having sick leave but, the younger pilots tend to have strong individualistic personalities.

    Malignant Melanoma in Pilots and Aircrew

    Abstract : Malignant melanoma is a malignant tumor of melanocyte, its incidence has been steadily increasing in the past several decades. It accounts for 160,000 new cases annually, with slightly more occuring in women than in men. Since it is common in circrew compared to general population, many studies have been done to figure out the development mechanism. Although its incidece in Korea is not high, it is important that we should understand and prevent it. The pilot with malignant melanoma was grounded in the past. But, recently they can fly according to their clinical stage.

    항공기 조종사의 안전한 비행을 위한 약물 복용 유의사항

    Abstract : Over-the-counter (OTC) drugs are usually approved for use in flight while some are not. The laboratory results showed that pilots are often motivated to consider the use of OTC medications. But those who do not have enough knowledge or have inappropriate information sometimes have to make proper decisions on OTC drug usage under more complicated constraints of a safety-critical job. To better understand how pilots come to use OTC drugs, this paper provides a summing-up suggestion for pilots’ decisions about using OTC drug products. In this paper factors were discussed that could influence drug trends and some guidance for safe medication were presented. In addition, the author argues that pilots should be provided with additional guidance when making decisions about OTC drug use.

    Abstract : Population based research suggests that in the United States between 44,000 and 98,000 patients die each year from preventable errors, making medical error the eighth most common cause of death. As early as 1994, the medical community began evaluating best practices from the aviation industry as a possible avenue for reducing errors that occur in operating rooms and trauma situations. Aviation crew resource management (CRM) training, based on effective communication and teamwork skills, may translate to healthcare through error management training. Aviation and healthcare have similar point that focused on safety and efficiency. Now we need to discuss on introducing CRM that was proved as one of the successful tool for reducing human error and increasing safety.

    운항관리사의 피로·직무 스트레스 요인에 관한 연구

    Abstract : Flight Dispatchers often experience fatigue and stress on the job due to work-related factors. Performance impairment resulting from fatigue and job-stress is an important concern for aviation safety. The goal of this research was to investigate fatigue and job-stress factors for flight dispatchers. The research involved an extensive literature review of job characteristic, fatigue and job-stress, human factors in the general and specific (airline operation) environment and surveyed fatigue and job-stress level of flagship carrier's flight dispatcher compared to the other occupation groups. Specially flight dispatchers reported that night work created high level of fatigue and job requirement induced high level of stress. In addition, implications for fatigue and stress management were discussed.

    Abstract : Middle-ear barotrauma is a common problem reported by aircrew members and passengers. Studies have shown that 20~50% of passengers report ear complaints during the flight or after landing. Barotitis media is defined as an acute or chronic inflammation caused by a pressure differential between the tympanic cavity and the surrounding atmosphere. The Problems caused by otitis media or sinusitis of aircrew during landing and take-off can be a threat to aviation safety. Therefore, the aviation medical examiner should know about barotitis media of aircrews. In this paper, we aimed to learn what is barotitis media and how much it could affect aircrews. Also, we aimed to evaluate how barotitis media could make an impact on crews during flight, and how to manage these problems.

    Abstract : The medical fitness of pilots is part of the civil aviation safety scenery. This study aimed to analyze the health problems occurring among Korean commercial pilots. Three data sources were used to identify the health problems: 7,574 aviation medical examination data, 5,400 sick leaves and 39 cases of a decrease in medical fitness during flight duty period and layover of Korean commercial pilots who have been working for K airline from Jan. 1, 2016 to Dec. 31, 2018. This study was retrospectively analyzed using SPSS 22 statistical program. Waiver of medical certificates was an average 11.9% of total issuance for 3 years, with a denial of 0.1%. The leading cause of denial of medical certificates was predominantly of cardiovascular cause (55.6%). Mild respiratory and digestive disease accounted for 82% of total sick prevalence and 68% of total sick days. The psychiatric and cardiovascular disease were ranked high according to the number of days lost per case. The most common cause of decrease in medical fitness during flight duty period was acute abdominal pain (36.4%) and musculoskeletal disorder (40%) when staying abroad. Aeromedical emphasis on minimizing cardiovascular risk remains appropriate. Major pilot health problems identified in this study should be considered in establishing pilot health promotion programs for aviation safety.

    Abstract : In accordance with ICAO SARPs, the states should implement appropriate aviation-related health promotion for airmen subject to an aviation medical examination to reduce future medical risks to flight safety. We aimed to analyze the results of the aviation medical examination to use as objective evidence for establishing an aviationrelated health promotion plan. The results of the final issuance of airmen medical certificate from 2015~2017 were collected from the e-sky computer system. The 30 cases (0.3%) in 2015, 22 cases (0.2%) in 2016, 38 cases (0.3%) in 2017 were unfit for flight duty. The 34 unfit out of the total 90 cases were renewal physical examinations. The cardiovascular disease was most commonly in 11 cases, followed by 8 tumors, 4 psychiatric diseases, 3 ophthalmic diseases, 2 respiratory diseases, 2 neurologic diseases, 2 otolaryngological diseases, 1 endocrine disease and 1 digestive disease. The results of this study can be used as objective data in aviation-related health promotion plan. The health promotion activities based on objective data can contribute to improving aviation safety by improving the lifestyles of airmen.

    Effects of Microgravity on Human Physiology

    Abstract : Space exploration is one of the dreams of humankind. However, the intriguing environment was a challenge for the human body, where we must counter with many extreme conditions such as thermal support, radiation, microgravity. Life, as well as the human body, developed and evolved in the continuous presence of gravity, especially when living creatures transfer from the ocean to the land. Once this gravitational force doesn't impact on the body, the drastic changes occur. Some of these changes were observed immediately, while others progress only slowly. Since the first orbital flight was performed, several hazards for the organs of the human body were identified [1]. These changes in human physiology can reverse when astronauts return to Earth. This article will review the published findings of the effects of microgravity exposure on the human body.

    Abstract : "The cabin environment has many physiological effects on commercial aircraft passengers and medical providers, and environmental stress factors exist. Therefore, it is important for medical providers to understand the effects of aviation physiology and cabin environment on the human body. It should also be remembered that these physiological changes and environmental stress factors can affect passengers as well as flight crew and also medical equipment. Providing medical assistance during a flight offers a number of unique challenges including lower cabin pressure, tight quarters, crowded conditions, and loud background noise. The purpose of this Korean guideline is to offer an overview on various in-flight emergencies that could be anticipated and to outline treatment priorities."

    Review of the History of Animals that Helped Human Life and Safety for Aerospace Medical Research and Space Exploration

    Abstract : In 2019, the Aerospace Medical Association of Korea celebrated its 30th anniversary. On the other side of the world, it was also the 62nd anniversary of Russian launch Sputnik 1 of the world’s first artificial satellite on October 4, 1957. In additionally, the world, especially the United States was shocked, when on November 3, 1957, Sputnik 2 blasted into Earth orbit with a dog named “Laika”; it was the role of veterinarian’s activities for aerospace medical research and exploration. Veterinarians (Vets) are responsible for the health of all the animals for aerospace medicine whether on the ground or in space. Vets can enhance animal and public health and this knowledge of Vets and astronauts can extend their mission durations, go to nearby Earth Asteroids, Mars and other heavenly bodies to study their living and non-living characteristics. This review article is the brief history of the original growth of the veterinarian’s activities for the aerospace medical research, in order to stimulate future strategies for improvements in the space life sciences and exploration.

    알레르기 비염 진단 및 치료, 항공업무 적합성 평가에 있어 항공전문의사의 역할

    Abstract : Since allergic rhinitis is a disease with a very high prevalence, it is common to find patients with allergic rhinitis among aviation workers. However, they are so afraid that the report of rhinitis will have a negative effect on the evaluation of one's work suitability. Therefore, aviation medical examiners (AMEs) must be able to accurately understand the nature of allergic rhinitis, and confidently explain that appropriate treatment of rhinitis has a positive effect on their performance. In the treatment of allergic rhinitis, there are some medications that may cause drowsiness, which may impair the accuracy and safety of the aviation service. Therefore, AMEs should accurately know safe drugs that do not cause drowsiness and prescribe them to patients. In addition, it is necessary to know exactly whether air workers may receive the latest treatments for allergic rhinitis, such as surgical treatment and immunotherapy, and be able to recommend these treatments appropriately. Therefore, in this paper, we first briefly describe the pathophysiology, genetics, causative antigen, symptoms, diagnosis, and treatment of allergic rhinitis. We also aimed to discuss safe medication and other treatment modalities for allergic rhinitis.

    피로위험관리를 위한 조종사 피로측정방법 고찰

    Abstract : Pilot fatigue is a great hazard to aviation safety. In recent years, longdistance fights have been increasing and flight hours have been increasing, which has made fatigue issues important. International organizations in the aviation sectors (ICAO, FAA, IATA etc.) recommend management of fatigue within the SMS (Safety Management System) framework. A scientific and systematic approach to measuring fatigue is required as a prerequisite to preparing safety management measures for pilot’s fatigue. Therefore, in this study, I would like to consider recent trends and implications for fatigue measurement. First, I aimed to consider recent the accident cases related to fatigue. Second, I also considered how to measure the pilot’s fatigue. Finally, the direction of the countermeasures against fatigue through fatigue measurement was developed and suggested.

    코로나바이러스감염증-19 (COVID-19)과 항공신체검사

    Abstract : Coronavirus disease 2019 (COVID-19) has had a significant impact on our society as a whole. The COVID-19 pandemic is not only a health crisis, it is also an economic, social and humanitarian crisis. Considering the dramatic global economic and social impact that the crisis has engendered, the aviation system is standing on the doorstep of rapid transformation. In particular, the impact on the aviation and travel industries is enormous. Air travel to most countries has been suspended and blocked. Looking at Korea’s current situation, COVID-19 has wholly changed the aviation industry. As COVID-19 spreads around the world, countries have come up with aviation safety measures. Infectious disease safety measures were established to protect passengers and crew members, and countries with collapsed medical systems extended the validity period for aviation medical examinations. In Korea, on August 11, the Ministry of Land, Infrastructure and Transport provided guidance on medical measures related to COVID-19 through an official letter of “Notification of cautions for pilots and air traffic control officers (ATCO) when COVID-19 is confirmed”. Overseas countries such as the United States and the United Kingdom have announced regulations for aviation medical examination regulations in relation to COVID-19, and have set standards for returning to aviation after COVID-19 is confirmed. In this paper, we would like to investigate the regulations for aviation medical examination related to COVID-19.

    항공 분야에서의 사물인터넷 기술 현황

    Abstract : Internet of Things (IoT) is a technology that communicates data between devices, which are things, using an embedded sensor connected through network backbone such as the internet. Here, data communication technology, sensor technology, and actuator (interface) technology are fused into IoT and it turns devices into smart things. As a result, vast sized data are being generated and that data is being processed into useful actions that can control the things that are devices to make our lives much fruitful. Nowadays, the IoT, early defined as Machine-to-Machine (M2M) connection, becomes a key technology powered by growing innovation of wireless communication trends in the internet connectivity through mobile networking.     최근 5년간 국적항공사 조종사의 항공신체검사 부적합 판정결과

    Abstract : This study aimed to quantify the causes of Korean airline pilots' medical disqualification during 2015~2019. The airline pilot disqualification data is important for grasping what health problems are common to pilots and establishing preventive health care programs. We were able to obtain data from official sources at the Korean Civil Aviation Authority during 2015~2019. The total number of pilots was 29,095, and the number of disqualified professional pilots in Korea was 36. Looking at airline pilots' status by age group, the 40s were the highest with 38%. However, the disqualification rate per 1,000 pilots was the highest in the 60s, at 4.6. When the different diagnostic categories classified the disqualification results, cardiovascular disease was the highest with 12 cases (33.3%), followed by eye disease, malignancy, neurology, psychiatry, otorhinolaryngology, and respiratory disease. The total number of pilots during 2015~2019 was 29,095, and the overall disqualification rate of 1.6/1,000 pilots.

    교대근무 항공정비사의 피로관리 연구

    Abstract : In the case of aviation maintenance work, several kinds of shift works are performed during day and night, and a lot of works are performed on the ramp due to the characteristics of the aircraft flight schedule. Maintenance workers are often exposed to the sun or in cold temperatures for aircraft maintenance works. The fatigue risk of the maintenance workers will be weighted. In particular, the work at night shift under normal biological rhythms can make maintenance workers feel sleepy during daytime work. It can also affect the safety of aircraft and individual workers. Accordingly, this study will consider fatigue-related previous researches in the field of aviation maintenance, review the effects of shift work, and how fatigue affects aviation mechanics during the day/night shifts. Considering these factors, we tried to present a plan that can minimize the fatigue of aviation workers through this study

    피로위험관리를 위한 조종사 피로측정방법 고찰

    Abstract : Pilot fatigue is a great hazard to aviation safety. In recent years, longdistance fights have been increasing and flight hours have been increasing, which has made fatigue issues important. International organizations in the aviation sectors (ICAO, FAA, IATA etc.) recommend management of fatigue within the SMS (Safety Management System) framework. A scientific and systematic approach to measuring fatigue is required as a prerequisite to preparing safety management measures for pilot’s fatigue. Therefore, in this study, I would like to consider recent trends and implications for fatigue measurement. First, I aimed to consider recent the accident cases related to fatigue. Second, I also considered how to measure the pilot’s fatigue. Finally, the direction of the countermeasures against fatigue through fatigue measurement was developed and suggested.

    항공종사자 건강증진활동계획에 반영되어야 할 근거자료: 3년간의 항공신체검사 결과 분석

    Abstract : In accordance with ICAO SARPs, the states should implement appropriate aviation-related health promotion for airmen subject to an aviation medical examination to reduce future medical risks to flight safety. We aimed to analyze the results of the aviation medical examination to use as objective evidence for establishing an aviationrelated health promotion plan. The results of the final issuance of airmen medical certificate from 2015~2017 were collected from the e-sky computer system. The 30 cases (0.3%) in 2015, 22 cases (0.2%) in 2016, 38 cases (0.3%) in 2017 were unfit for flight duty. The 34 unfit out of the total 90 cases were renewal physical examinations. The cardiovascular disease was most commonly in 11 cases, followed by 8 tumors, 4 psychiatric diseases, 3 ophthalmic diseases, 2 respiratory diseases, 2 neurologic diseases, 2 otolaryngological diseases, 1 endocrine disease and 1 digestive disease. The results of this study can be used as objective data in aviation-related health promotion plan. The health promotion activities based on objective data can contribute to improving aviation safety by improving the lifestyles of airmen.
